PART 1

The Nutritional Therapy Descriptor

 

Nutritional Therapy is the application of nutrition science in the promotion of health, peak performance and individual care. Registered Nutritional therapy practitioners use a wide range of tools to assess and identify potential nutritional imbalances and understand how these may contribute to an individual's symptoms and health concerns. This approach allows them to work with individuals to address nutritional balance and help support the body towards maintaining health. Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing.

 

Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a 'one size fits all' approach. Practitioners never recommend nutritional therapy as a replacement for medical advice and always refer any client with 'red flag' signs or symptoms to their medical professional. They will also frequently work alongside a medical professional and will communicate with other healthcare professionals involved in the client's care to explain any nutritional therapy programme that has been provided.

 

The Registered Nutritional Therapy Practitioner (RNTP) requests that the Client notes the following:

 

  • The degree of benefit obtainable from Nutritional Therapy may vary between clients with similar health problems and following a similar Nutritional Therapy programme.

  • Nutritional advice will be tailored to support health conditions and/or health concerns identified and agreed upon between both parties.

  • RNTPs are not permitted to diagnose or claim to treat medical conditions.

  • RNTPs' recommendations are not a substitute for professional medical advice and/or treatment.

  • Your RNTP may recommend food supplements and/or functional testing as part of your Nutritional Therapy programme and may receive a commission on these products or services.

  • Standards of professional practice in nutritional therapy are governed by the Complementary and Natural Healthcare Council (CNHC) Code of Conduct, Ethics and Performance

  • This document only covers the practice of nutritional therapy within this consultation, and your practitioner will make it clear if he or she intends to provide services that are outside the scope of nutritional therapy practice.

 

The Client understands and agrees to the following:

 

  • I am responsible for contacting my GP about any health concerns.

  • If I am receiving treatment from my GP, or any other medical provider, I should tell him/her about any nutritional recommendations provided by my RNTP. This is necessary because of any possible reaction between medication and the nutritional programme.

  • It is important that I tell my RNTP about any medical diagnosis, medication, herbal medicine, or food supplements I am taking, as this may affect the nutritional programme.

  • If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact my RNTP promptly for clarification.

  • I understand that the recommendations are personal to me and may not be appropriate for others.

  • I must contact my RNTP should I wish to continue any specified supplement programme for longer than the original agreed period, to avoid any potential adverse reactions.

  • Recording consultations using any form of electronic media is not allowed without the written permission of both me and my RNT

  • I agree that all content provided by Live Well Nutrition will not be reproduced or shared with any other party.

  • The Nutritional Therapy Questionnaire and 5-day food diary should be returned to Live Well Nutrition no later than 48 hours before the initial consultation. Failure to return may result in the consultation being postponed.

 

Confidentiality and Data Protection

 

The Registered Nutritional Therapy Practitioner will keep your personal information confidential and secure following the UK GDPR guidelines for the practice. The RNTP will not share your information with third parties without your consent. However, if the RNTP believes there is a risk of significant harm to yourself or another person, the RNTP may pass the information onto an appropriate authority using the legal basis of vital interest. A separate Privacy and Consent Notice is available for your review.


 

PART 2

Payment Terms and Cancellation policy

 

Fees & Payment

  • All session fees are as stated on the Live Well Nutrition website and are agreed upon at the time of booking.

  • Payment is required in full at the time of booking to secure your appointment or programme.

  • For services offered on a payment plan, the first instalment is due at the time of booking. Subsequent payments must be made on the agreed-upon dates as specified in your payment schedule.

  • Payments can be made by credit/debit card or bank transfer.

  • If a recurring payment fails, is declined, or is not made on the agreed-upon date, services will be suspended until the outstanding balance is settled.

 

Right to Cancel ('Cooling-off Period')

  • In accordance with UK law, you have a 14-day "cooling-off" period to cancel any package or programme, starting from the date of your purchase. Cancellations under this policy must be made in writing by emailing info@livewellnutrition.co.uk

  • A full refund will be issued within 14 days of cancellation, provided that no services have commenced within the 14-day period.

  • If the service has commenced at your request within the 14-day cooling-off period, you will be refunded for the portion of the package that has not yet been provided. The amount deducted will be proportionate to the services already delivered.

 

Appointment Cancellation & Rescheduling Policy

  • A minimum of 48 working hours' notice is required to cancel or reschedule an individual appointment. (Monday-Friday, 9am-5pm, excluding UK bank holidays)

  • Notice must be provided directly via the booking system or email info@livewellnutrition.co.uk

  • This 48-hour policy is separate from the 14-day "cooling-off" period, which applies to the purchase of the entire package.

  • If cancellation is instigated by Live Well Nutrition we will provide you with as much notice as possible. We will offer to reschedule the appointment to a mutually convenient time. 

 

Late Cancellations & Failure to Attend an Appointment

  • Cancellations of an individual appointment made with less than 48 working hours' notice, and any failure to attend a scheduled appointment ('no-show'), will result in the full fee for that specific appointment being charged.

  • For clients on a programme or package, a late-cancelled or missed appointment will be forfeited from the package. The session cannot be rescheduled or refunded.

 

Programme Terms

​​Early Termination of Programmes

Outside of the 14-day statutory 'cooling-off' period, all payments for programmes are non-refundable. When you purchase a programme, you are committing to the full term. No partial refunds will be issued should you choose to terminate the service before the programme is complete. In cases of exceptional unforeseen circumstances (such as a serious medical issue), please contact us to discuss your situation5

 

Time frames 

All sessions included in a package must be used within a specified timeframe to ensure continuity and best results. The 6-week programme must be completed within 8 weeks of the first session. The 3-month programme must be completed within 4 months. The 6-month programme must be completed within 8 months. Any sessions not used within this period will be forfeited and are non-refundable
